The Child Death Review is now a prominent part of the updated Working Together to Safeguard Children. It is advisable this document is read alongside Working Together and kept at hand for safeguarding practitioners.
This statutory guidance applies to all organisations involved with the process of child death review, and should be complied with unless exceptional circumstances arise.
All professionals who care for children, or who have a role in the child death review process, should read and follow this guidance so that they can respond to each child death appropriately.
This document attempts to clarify processes and set out high-level principles for how professionals across all agencies involved in the child death review process should work together.
This guide covers the legislative requirements and expectations for all services that have a duty of care for children.
The guidance sets out the process in order to:
- improve the experience of bereaved families, and professionals involved in caring for children
- ensure that information from the child death review process is systematically captured in every case to enable learning to prevent future deaths
